We need your correct name, address and phone number to stay in touch with you about your health care. If any information changes, please contact:

  • Your local county social services agency or Social Security Administration
  • United States Post Office
  • The member services department of your health insurance plan (see below)

Your health insurance plan will send your updated information to us. 

If you lose your Medi-Cal benefit card, call or visit your local county social services agency. If you lose your health insurance plan card, call your plan's customer service department at the numbers below.

Your medical information is protected by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). According to HIPAA, we must make sure that you are a member of our medical group.

If you are not a member or are calling on behalf of your spouse or child, we may need permission to speak with you. If you need a friend or relative to speak on your behalf, please fill out the disclosure of protected health information form.

To get this form, call your health insurance plan's customer service department (see numbers below) and ask them to send you a copy.

If you lose your Medi-Cal benefit card, call or visit your local county social services agency. If you lose your health insurance plan card, call your plan's customer service department at the numbers below.

If you need medical care fast but don’t know if you should go to an emergency room or urgent care, call your PCP first. If you need urgent care, find out if you can see your doctor. 

  • If your doctor isn’t available, go to the best urgent care location for you.  
  • If it's after hours, the office voicemail will have directions for you to follow.
  • If it’s a true emergency, call 911 or go to the nearest ER right away.

Don't go to a hospital emergency room for general health care services. Call your PCP instead. 

If you want to see a specialist, talk to your doctor. They will suggest a specialist based on your medical problems and who's in the network.

Before you see specialists and get medical services and medical supplies, get approval from your medical network. To find out if a specialist is part of your medical network, call your insurance plan (see numbers below).

Billing and copays

If you get Medi-Cal covered benefits through a Medi-Cal provider, there are no copays, premiums or out-of-pocket expenses.

If you get care from your health insurance plan Medi-Cal provider, you should not be billed for any covered services. If you do get a bill, call the provider and give them your Medi-Cal ID and/or other insurance information.

If you get another bill or are sent to collections, call the customer service department of your health insurance plan at the numbers below.
