About your medical information

  • We need your correct name, address and phone number to stay in touch with you about your health care. If any of this information has changed, please contact:

    • Your local county social services agency or the Social Security Administration
    • United States Post Office
    • The member services department of your health insurance plan

    Your health plan will send your updated information to Inland Faculty Medical Group (IFMG).

  • If you lose your Medi-Cal benefit card, call or visit your local county social services agency. If you lose your health insurance plan card, call your plan's customer service department at the numbers below:

  • We understand that medical information about you and your health is personal. We're committed to protecting it and your privacy.

    Your medical information is protected by the Health Insurance Portability and Accountability Act of 1996 (HIPAA). According to HIPAA, we must make sure that you are a member of our medical group.

    If you are not a member or are calling on behalf of your spouse or child, we may need permission to speak with you. If you need a friend or relative to speak on your behalf, please complete the disclosure of protected health information form.

    If you prefer, call the customer service department of your health insurance plan and ask them to mail this disclosure to you.

Doctors and medical network

  • You can find your doctor's phone number on your health insurance plan card. Or contact:

  • You may change your primary care doctor for any reason every 30 days. Your medical group's member services department can help with that. Their number is on your health insurance plan ID card. To make sure you can see the same specialist(s), choose a doctor with IFMG.

  • If you need medical care quickly but don’t know if you should go to an emergency room or urgent care, call your doctor first. If you need urgent care, find out if you can see your doctor. 

    • If your doctor isn’t available, you’ll be directed to the best urgent care location for you. 
    • If it's after hours, the office voicemail will have directions for you to follow.

    Do not go to a hospital emergency room for general health care services. Call your doctor instead.

  • If there’s a certain specialist you’d like to see, talk to your doctor. He or she will recommend a specialist based on your medical condition and suggest available specialists in the network. 

    Your medical network is responsible for authorizing referrals to specialists, medical services and medical supplies. To find out if an IFMG specialist is part of your medical network, please call:

Billing and copays

  • If you receive Medi-Cal covered benefits through a Medi-Cal provider, there are no copayments, premiums or out-of-pocket expenses.

  • If you receive care from your health insurance plan Medi-Cal provider, you should not be billed for any balance due on Medi-Cal covered services. If you do get a bill, get in touch with the provider and give them your Medi-Cal ID and/or other insurance information. 

    If you get another bill or are sent to collections, call the customer service department of your health insurance plan at the numbers below:
